FIRST STEP HOME PROVIDER SERVICE AND SUPPORT PLAN
Mission: To create compassionate pathways for women’s substance use disorder recovery through an integrated continuum of services to achieve self-sufficiency.
Vision: First Step Home will be the center of excellence in providing care for women and women with children suffering from the effects of substance use disorder.
Goals: To provide evidence-based programs for women to achieve recovery. To provide services with collaboration, integrity, respect, and teamwork.
Residential Treatment: First Step Home provides Residential Treatment, Level of Care 3.5, based on the American Society of Substance use disorder Medicine (ASAM) standards. The Residential Treatment Program provides 24/7 supervision and support to stabilize the disease of substance use disorder and create a foundation for recovery. Individual and Group Counseling Services are provided daily, as well as additional medical and recovery-based services to include Withdrawal Management (LOC 3.2, ASAM), Mental Health Services, Medication Assisted Treatment (MAT), Nursing, and an onsite Primary Health Care Clinic. Women are allowed to bring children with them to treatment, up to the age of 12 years old.
Goals and Scope: Residential Treatment offers group and individual counseling, medication maintenance, medical nursing support, childcare services, case management services, and more. Services are designed with the understanding that substance use disorder is a treatable disease, and that people can recover and use their strengths to lead productive and fulfilling lives.
The following Residential services are available: Residential/Withdrawal Management 24/7, Case Management Monday-Friday 9am-5pm, Peer Support Monday-Friday 9am-5pm.
Needs and Characteristics of Population Served: Residential 3.2 and 3.5 programs serve women who need a safe and stable living environment, have significant social and/or psychological problems. This level of care assist women who’s substance use disorder needs a 24-hour supportive treatment environment to establish or continue a recovery process.
Intensive Outpatient ASAM 2.1: The Intensive Outpatient Program (IOP) provides group and individual counseling. Clients receive access to case management during this program as well. Intensive Outpatient can be a step-down program after residential stabilization, or clients can enter directly into IOP based on a clinical assessment. MAT is also provided during this level of care, based on clinical and medical necessity.
Goals and Scope: The Intensive Outpatient Program (IOP) assists women in managing their substance use disorder and mental health needs with individualized treatment plans based on the unique strengths, needs and abilities of each client. Treatment services include housing, individual, group and family therapy, Maternal Substance use disorder Program support, medication management, medical nursing support, case management, vocational training, and peer support services.
The following Intensive Outpatient (IOP) services are available: Intensive Outpatient services Monday-Friday 9am-5pm, Case Management Monday-Friday 9am-5pm, Peer Support Monday-Friday 9am-5pm.
Needs and Characteristics of Population Served: The Intensive Outpatient program serves women who need ASAM level of care 2.1 and need 9-19 hours of structured treatment per week. Treatment services are for women whose substance use disorder symptoms are to severe for outpatient services but do not require 24-hour residential care.
Outpatient: Outpatient Treatment (OP) provides a combination of individual and group therapy dependent on clinical assessment. MAT is also available during Outpatient Treatment.
Goals and Scope: Our Outpatient Treatment (OP) program provides a variety of services that enable clients to maintain sobriety and remain drug free. These services allow individuals to focus on relapse prevention, education and wellness, resiliency, and maintaining sobriety. Outpatient Treatment clients live in the community and come to First Step Home for individual, couples, group and family counseling, as well as supportive case management and Child Resiliency Program services.
The following Outpatient (OP) services are available: Outpatient services Monday-Friday 9am-5pm, Case Management Monday-Friday 9am-5pm, Peer Support Monday-Friday 9am-5pm.
Needs and Characteristics of Population Served: The Outpatient program serves women who need ASAM level of care 1.0 and need less than 9 hours per week, no acute withdraw risks, stable physical and mental health, and a safe/supportive home environment.
Case Management: Case management services provides women with outside services to eliminate any psychosocial barriers to recovery. Case managers can assist women to address medical needs, legal issues, employment, as well as other additional services and promotion of life skills.
Goals and Scope: Case Management services to clients who are in need of continued support in the community. Case Managers work in the community, assisting clients with any psychosocial barriers that may impact a successful recovery. Specific areas that are addressed and assisted with include, employment, permanent housing, medical care, mental health support, daycare vouchers, food stamps, state-issued benefits, and many more.
The following Case Management services are available: Case Management services Monday-Friday 9am-5pm.
Needs and Characteristics of Population Served: Women who need require Case Management assistance with specific areas that are addressed and assisted with include, employment, permanent housing, medical care, mental health support, daycare vouchers, food stamps, state-issued benefits, and many more.
Recovery Housing: First Step Home’s Recovery Housing Program offers women and their children a safe, supportive and sober place to live while they continue to engage in outpatient services. Our Recovery Housing is managed by Housing Case Managers and programming includes activities and services to promote independent living.
Goals and Scope: Recovery Housing continues to make headway in its mission of providing quality housing opportunities for the women it serves. Women can establish a rent history and live on their own with support.
Recovery Housing Admission is available: Admission available Monday-Friday 8am-8pm, Staff onsite Monday-Saturday 8:30am-5pm.
Needs and Characteristics of Population Served: Women who need recovery housing are overcoming substance use disorders who lack a safe, stable, or substance-free home environment.
Maternal Nursing Services: First Step Home provides specific nursing services to address the needs of pregnant women and new moms recovering from substance use disorder, including a focus on ensuring that their babies are born healthy and have access to services and healthcare.
Goals and Scope: Maternal Substance use disorder (MA) is specially designed to meet the needs of pregnant or postpartum women in the early stages of sobriety. The Maternal Substance use disorder Program provides evidence-based wraparound services to address women's needs in recovery for substance use disorder and mental health issues. The program provides trauma-informed individual/group therapy, access to medical and prenatal care, and parenting and vocational education classes.
Maternal Nursing Services are available: Services available Monday 6-2pm, Wednesday-Friday 7am-3pm.
Needs and Characteristics of Population Served: Women that are pregnant and new moms recovering from substance use disorder, including a focus on ensuring that their babies are born healthy and have access to services and healthcare.
Peer Recovery Support Services: Peer Support Services are offered throughout all levels of care at First Step Home and provide access to clients to engage with others with lived experiences to share a message of strength, experience and hope.
Goals and Scope: Peer Support Services include ongoing support of recovery, resiliency and wellness needs, Supporting individuals in achieving goals through increased knowledge, skills, and connection, Supporting the development of life skills, and much more.
Peer Recovery Support Services are available: Peer Support Monday-Friday 9am-5pm.
Needs and Characteristics of Population Served: Women who need peer recovery support are navigating substance use disorder, mental health challenges, and/or trauma who may benefit from non-clinical, emphatic support by others with similar lived experiences.
Other Providers
Primary Care Healthcare/Dental Provider – HealthPoint: The purpose of Primary Care Services is to provide a foundational and established provider for physical and dental healthcare needs.
Goals and Scope: Goals are to provide general health assessment, discussion of concerns, and identification of potential issues like high blood pressure, diabetes, and other health related issues. Regular health screenings are essential for early detection and treatment of diseases, potential improving outcomes and quality of life.
Primary Care and Dental Services are available: Onsite at First Step Home Tuesday 1pm-4pm. Offsite Primary Care Services are available by appointment at HealthPoint (1401 Madison Ave, Covington, KY 41011). Dental appointments are available Wednesday 1pm-3pm (scheduled by First Step Home staff)
Needs and Characteristics of Population Served: Women who need primary care include women who are seeking routine and/or preventative healthcare.
Methadone Services; Northstar: Combines FDA-approved medications with counseling and behavioral therapy to treat substance use disorders, particularly those involving opioids and alcohol.
Goals and Scope: This approach helps manage withdrawal symptoms, reduce cravings, and lower the risk of overdose.
MAT Services are available: 311 Maritn Luther King Drvie East, Cincinnati, OH 45219 Hours Monday-Thursday 8am-6pm, Saturday 7am-10:45am.
Needs and Characteristics of Population Served: Women who need methadone are diagnosis with opioid use disorder to help manage withdrawal symptoms, reduce cravings, and lower the risk of overdose.
